I'll go with the scored disk as a culprit. Mine were starting to do the same after some riding offroad in the wet weather. Pays to clean the calipers after riding in mud as this is usually why pads & discs get scored. My disc was low too so this wouldn't have been helping. I have now fixed that problem.
On the other hand could your rear brake master cylinder be loose? That will cause that to happen & was half my problem.
Check that the piston/plunger is returning & working correctly although this isn't that likely to be the problem.
Also it would be good to check your brake pedal pivot bolt as this may be worn or loose & clevis pin (PIN, CLEVIS) ( part number 17) as this may be worn or rusty. Apply lithium grease to all pivot points.
Just one more thing....If your brake fluid hasn't been changed in awhile i would be doing this promply. Brake fluid only last about a year (or two if not used much) at the most for optimum lubrication/pressure.The brake lines could be gunked up.

Make sure you have flushed & bled the rear brake before adjusting the specs (if it's been more than a year or so since you have done this) . Read the warning & caution below.
To adjust the pedal position just loosen the lock nut on the piston/plunger & tighen to spec. (if needed after doing the above)

My rear brake has been getting worse lately, it locks on the dirt way to easy and on the blacktop as well if your not careful
It never used to be like that, the pads have 2-3mm left
I removed, cleaned and regreased the brake lever pivot bolt but that didn't help, it is smoother though
The piston moves in easy and the caliper seems to slide on the pins OK, it could probably do with cleaning and greasing though, no corrosion
the disk is scored but not to bad, there seems to be a few high spots on the disk where the pad is contacting but missing the rest, it's not bent or buckled
I'm suspecting the pads and the scored disk ?
Any thoughts/solutions anyone ?
I did a search but can't find any posts for this problem
